Practical Notes

  • Scoring approach matters: Respiratory event scoring criteria (AASM 2012 recommended vs. alternative rules, 3% vs. 4% desaturation criteria) influence AHI and derived metrics. Ensure consistency between the scoring used in your study and the thresholds applied here.
  • Position and stage interact with endotypes: A patient may exhibit predominantly anatomical OSA in supine NREM sleep but have REM-related patterns suggesting poor muscle compensation. Consider the interplay between positional and stage-dependent data when interpreting results.
  • Combine multiple surrogates when physiological (PUP) analysis is unavailable: No single PSG surrogate perfectly maps to a pathophysiological trait. Confidence increases when multiple surrogates point in the same direction. Always correlate with clinical findings.
  • These are surrogate measures: Definitive endotyping requires overnight physiological analysis (e.g., CPAP drop technique). The estimates provided here are based on clinically accessible surrogates with published predictive values.
  • Clinical judgement remains paramount: This calculator is a decision-support tool. Treatment decisions should integrate endotyping results with patient symptoms, preferences, comorbidities, and clinical expertise.
